B. R. WILLIAMS PASSENGER PROGRAM
Revised June 13, 2008 1. Passenger Program will be available from March 1 through October 31 of each year for entire United States. South only in other four (4) months. 2. Company can allow one (1) passenger at a time per trip. Passenger must comply with company seat belt policy at all times during trip. 3. Drivers will be allowed to carry a passenger any time during the eight (8) months. 4. B. R. Williams’ driver must be a minimum of 25 years old. 5. Passenger must be a minimum of 10 years old. (Exception: No one under 18 allowed on Honda property). 6. Driver must drive a minimum of six (6) months with B. R. Williams before being qualified for program. 7. Driver will be disqualified from Passenger Program for six (6) months if he has any one (1) preventable accident over $500.00 or any two (2) preventable accidents regardless of amount in the previous 12 months. 8. Passenger must be related to driver by one of the following: spouse, son or daughter, brother or sister, parent, legally adopted child or grandchild. 9. Travel is limited to continental United States. Trips into Canada cannot be allowed. 10. Passenger must obtain written authorization from company and must carry written authorization and a photo for I.D. purposes. This photo can be either a drivers’ license or a regular photo approved by Safety Department with signature to be kept during entire trip. If the grandchild is under the age of 18, the parent or legal guardian must sign paperwork on behalf of the child. 11. Passenger must also be covered by company’s health insurance or provide proof of coverage of another health insurance coverage to B. R. Williams. 12. The passenger must be covered by a separate insurance policy for the purpose of accidental death and $10,000 primary health insurance. This policy shall be acquired through B. R. Williams per below. 13. Approval and all paper work regarding trip must be completed prior to trip. There must be sufficient time to complete paperwork and get approval. 14. Driver will be expected to treat trip as if it is a single operation making allowances for time of deliveries and rest in sleeper as generally required under normal circumstances. 15. Passenger should not be allowed at any time to sit in drivers’ seat nor operate any of B. R. Williams’ equipment. Passenger should also never be allowed to assist in loading, unloading, etc. There cannot be any exceptions to this rule. 16. Driver is expected to comply with all terms of program; such non-compliance shall result in revocation of privilege or possibly dismissal depending on circumstances. Unauthorized passengers shall result in immediate dismissal of B. R. Williams’ driver. STEPS TO PARTICIPATE
1. Contact Safety Department to determine eligibility. 2. Contact dispatch to coordinate trip for availability. 3. After successful completion of steps 1 and 2, complete application for trip and insurance form. Passenger photo I.D., either drivers’ license or photo generated by the Safety Department with signature, and proof of health coverage must be produced at this time. 4. After approval of application by Safety Department and one (1) other staff person, driver must pay for insurance required above. Rates are as follows: 1 - day premium $5.50 3 - day premium $12.00 5 - day premium 19.00 7 - day premium 24.00 10 - day premium 32.00 14 - day premium 40.00 It shall be determined by company which above coverage must be obtained after consideration of potential length of trip. Payment for premium must be paid in cash or check made payable to B. R. Williams Trucking, Inc. B. R. Williams is collecting the exact amount to be remitted to the insurance company so that your insurance is in effect immediately at the beginning of your trip, so that your payment is reimbursing the company for amounts already paid. The B. R. Williams Trucking, Inc. Passenger Program is a privilege and may be changed or revoked at any time without notice. Continuation of this program is dependent on our entire fleet’s as well as individual’s safety performance and our continued close relationship with our property/casualty and health insurance companies so: PLEASE DRIVE SAFELY
